Processes and ensures storage of health information in electronic medical record or chart as appropriate. Analyzes medical records. Identifies documentation deficiencies. Performs medical records review activities. Summarizes and reports results of review activities to hospital administration, medical staff and Health Information Management (HIM) Committee.
Essential Responsibilities:
- RECORD ANALYSIS
- Analyze post-discharged inpatient and designated hospital-based outpatient records for documentation deficiencies. Follows Medical Staff By-laws, reimbursement requirements and The Joint Commission (TJC) standards relative to completion of medical records.
- Productivity: Analyze a minimum of 13 records per hour.
- Quality standards: 95% accuracy rate.
- Turnaround time to Analyze medical records: 5 days.
- Notifies practitioners on delinquent/incomplete records on a bi-weekly basis. Monitor the delinquent records are completed by physicians in 14 days. Assist physicians with chart completions when necessary. Identifies physician deficiencies, data discrepancies and report out delinquent records.
- BIRTH CERTIFICATE PROCESSING
- Completes and processes a birth certificate for each live birth within the required 5 day timeframe.
- Interview mother to help complete Birth Certificate worksheet, data entry into State Vital Statistics department, and provide copy/receipt to mother.
- Productivity: Complete birth certificate within a minimum of 1.5 per hour and 5 day timeframe.
- Quality standards: 100% accuracy for the completed birth certificates.
- RECORD REVIEWS
- Conducts monthly medical record quality reviews for 50% (sample size) of total discharges.
- Recognizes areas for improvement and skills enhancement. Summarizes and reports results to hospital administration, medical staff, and Health Information Management (HIM) Committee.
- Productivity: a minimum of 10 quality medical record reviews per hour.
- Quality standards: 95% accuracy rate.
- GENERAL DUTIES
- Verifies daily census report for admissions and discharges.
- Correct and/or update member and patient information, when needed.
- Performs other duties as assigned.
Basic Qualifications:
Experience
- Minimum one (1) year medical records experience or medical records training or acute hospital setting.
Education
- High school diploma. Post high school coursework in medical terminology, medical records or equivalent related experience.
License, Certification, Registration
- N/A
Additional Requirements:
- Demonstrated knowledge of and skill in adaptability, data analysis, customer service, oral communication, problem solving and systems thinking, teamwork and written communication.
- Demonstrated knowledge of and skill in database PC applications.
Preferred Qualifications:
- Knowledge of routine hospital procedures, medical records and confidentiality.
- Knowledge of The Joint Commission (TJC) rules and regulations for acute hospital settings.
- Formal training in basic electronic medical records.
- Registered Health Information Technician (RHIT) preferred.
- Proof of medical terminology course completion and 45wpm typing.
