ProMedica Hospice, formerly Heartland Hospice, is currently searching for a full-time Medical Social Worker to support their hospice patients in the Rockford and surrounding areas.
The Medical Social Worker is responsible for providing patients and families psychosocial care regarding economic, social, and emotional needs. This position requires a Masters in Social Work and social LSW or LCSW in IL. WI licensure will be required shortly after hire.

Educational Requirements
Masters Degree in Social Work from an accredited institution.May require licensure or certificate as required by the state, and in good standing. (if applicable)
Position Requirements
Prefer one year experience in a health care setting. 100% travel necessary on a daily basis. Demonstrate problem solving and decision making skills. Demonstrated ability to work will with an interdisciplinary team. Excellent communication skills. Ability to develop and maintain rapport with the client and family.
