SUMMARY:

The Memory Care Director is responsible for coordination of all resident care and staffing of Memory Care services. The Memory Care Director will provide input related to pre-admission assessments and decision-making, reassessments and care plans. The Memory Care Director will offer dementia-related expertise to assist the resident and his or her responsible party in maintaining the physical and emotional health of the residents. The Memory Care Director will liaison with hospital personnel, physicians, community organizations (to include the local Alzheimer's Association Chapter) and other health related service agencies to provide services to the residents, support to families and training staff on an ongoing basis. Additionally, the Memory Care Program Director will be responsible for the interviewing, hiring, evaluating, coordinating, motivating, monitoring performance, scheduling and supervising the memory care department staff. They will also make recommendations to the Healthcare Director with respect to staff evaluations, merit pay increases, reprimands, and disciplinary actions.

GENERAL AREAS OF RESPONSIBILITY:
  • Coordinate care for all residents by creating an environment and flow to the day that matches each resident's physical and cognitive needs.
  • With support from social programming, ensure all memory care staff are trained to provide programming and activities to ensure proper engagement (activities) at all times.
  • Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of residents as needed.
  • Participate in community's resident assessment program in accordance with company policy and state regulations. Provide input to Healthcare Director regarding Memory Care resident pre-admission assessments and reassessments.
  • Coordinate departmental schedule and ensure adequate staffing in accordance with company standards, policies, procedures, budgets and state law.
  • Arrange for coverage of job duties within the department during employee absences through either delegation or personal completion of duties.
  • Responsible for ensuring MC staffing is up to date on OnShift.
  • On call rotation with Healthcare Director and Assisted Living Director.
  • Interface with pharmacy/family to ensure orders are accurate.
  • New hire orientation – ensure process is finalized and coordinate duties effectively.

QUALIFICATIONS:
  • Graduate of an accredited program for licensed vocational/practical nurses
  • Current Texas LVN license.
  • Knowledge of Alzheimer's and other types of dementia; provide documentation on certifications and related continuing education
  • At least two-years of experience working with residents with dementia
  • Be passionate to providing a high quality of customer service and resident satisfaction
  • Organized, excellent communication, follow through, able to work as a part of a team and solution orientated
