Palliative Care Nurse Practitioner Job Requirements Include:

  • Evaluate patients (existing, new and prospective) assessing their needs, limitations, and values
  • Address patient concerns and goals while maintaining constant communication with the patient as well as their family as is appropriate
  • Build rapport with patients and their families
  • Develop a long term plan of care for patients
  • Plan, develop, organize, implement, evaluate, and direct services with input from patient and family
  • Works in collaboration with other physicians and Hestia Palliative Care and Hestia Hospice team in patient evaluation and treatment, to further their understanding of significant social and emotional factors.
  • Assists patients with adjustment to treatment, including problems of depression, anxiety, denial and/or anger.
  • Serves as patient advocate and liaison with physicians, families, and community to ensure continuity of care.
  • Documents history, assessments, and interventions to reflect a comprehensive and integrated approach to patient care
