Location: SE Portland, on site

Compensation: $19.38 - $21.42/ hour

Housing Transitions Program (HTP) is a relationship-centered program that utilizes the Housing First model. HTP assists in removing barriers to permanent housing through case management services of housing placement and retention, employment services, and outreach to self-identified women experiencing homelessness who are 25 and better and who do not have minor children in their care. In addition to case management and outreach, HTP also operates a morning drop-in center where self-identified women can access immediate resources and community with their peers.


Who You Are:

The primary focus of the Part Time Outreach Case Manager is to increase the program ability to reach the most vulnerable women who are living outside, and to help faith and business community members respond in authentic and helpful ways to their houseless neighbors.


What You Will Work On:

  • Participate in drop-in day space duties 1 day per week.
  • Ensure the integration of HTP services with other Catholic Charities programs.
  • Demonstrate responsibility for safety of clients, all assigned staff, and property; assist in crisis situations as appropriate; follow all established safety protocols and procedures; possess working knowledge of fire regulations and evacuation procedures.
  • Assume limited caseload of clients providing direct case management, retention, advocacy, and support services with the goal of finding and securing permanent housing specifically clients met on outreach, who utilize HTP drop-in or reside in a Catholic Charities sponsored village.
  • Facilitate access to needed community services such as employment, physical health and mental health services, benefits assistance, and addiction treatment.


What You Bring:

Education, Certifications, and Licensure Requirements/Preferences

  • Bachelor degree (4-Year Program) or equivalent lived and or worked experience, required.

Experience Requirements/Preferences

  • 1-year direct work with individuals experiencing homelessness preferred.
  • 2 years experience providing case management, information, referrals, and advocacy.
  • Trainings and Specialized Knowledge (If not currently certified, you must be willing to be certified within the first 6 months of employment)
    • Current CPR/First Aid certifications.
    • Narcan training
    • Blood Borne Pathogens
    • Mental Health First Aid certified.
  • Experienced in crisis intervention and de-escalation.


VACCINATION POLICY

Catholic Charities of Oregon requires all employees to confirm they are fully vaccinated, or in the process of receiving the COVID-19 vaccination.
