Case Manager / Personal Agent

This position is for a Case Manager (called a Personal Agent) serving adults who experience intellectual/developmental disabilities (I/DD) in Douglas County.
As a Personal Agent, you will have the opportunity to make a positive impact in the lives of those we serve. Through a person-centered planning process, you will work with customers (the individuals we serve) to plan, develop, and implement individualized support plans based on their needs, goals, and dreams.
Personal Agents must be able to:
- Demonstrate effective documentation techniques.
- Communicate effectively (both oral and written).
- Plan and facilitate/coordinate meetings involving customers, their families, local agencies, and community partners.
- Work independently and efficiently to organize time and meet deadlines.
- Remain calm and use good judgment during confrontational or high-pressure situations.
- Make decisions independently in accordance with established policies, procedures, and Oregon Administrative Rules.
- Facilitate Medicaid services for persons with I/DD in accordance with Federal, State, and the organization’s established rules, policies, and procedures.
Minimum Requirements:
- A bachelor's degree in behavioral science, social science, or a closely related field; or a bachelor’s degree in any field and one year of human services related experience; or an associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience; or three years of human services related experience.
- Knowledge of the public service system for developmental disabilities services in Oregon.
- Proficient computer skills including experience with MS Office Suite.
- Excellent verbal and written communication skills.
- Strong organizational and problem-solving skills.
- Valid Oregon Drivers License.
- Pass a background check.
- Residence in Oregon and within county/counties served (or reasonable distance).
Compensation & Benefits:
$28.56 per hour with incremental increases
