RHHD is currently seeking a Population Health Manager with a dedicated interest in population health who can perform work without prior approval from supervisor; independence of action and judgement is expected. Seeks guidance when dealing with controversial and/or politically sensitive issues, actions that involve district program policy and procedure changes, and personnel issues that could result in discipline. The Population Health Manager will direct and support our efforts in a variety of areas of public health concern, including substance use disorder, violence prevention, and health equity.

This Population Health Manager provides or coordinates health improvement planning services in the local health district by planning, implementing, and evaluating health education activities, working closely to fulfill health improvement goals, preparing and submitting grant applications related to health improvement projects, delivering educational programs to clients and staff, conducting health education trainings, revising and developing educational materials, collecting program-specific data and writing reports, conducting needs assessments and facilitating the CHIP process, providing technical assistance to local and community agencies/organizations regarding program planning, serving as public contact or PIO about services and activities and preparing news releases and coordinating district media contacts and activities. May include the supervision of staff. Serves as a member of the district's Management Team working closely with the Health Director and Team to achieve the VDH mission.

This Population Health Manager will primarily work out of our Richmond office but will also spend time in our Henrico County offices. Required to participate in district events, meetings, emergency operations, etc. Mileage reimbursement is available for any work-related travel. The typical work schedule is Monday-Friday, 8:00 am – 4:30 pm. May require work during some weekend and/or evening hours.

Minimum Qualifications

  • Knowledge of principles and practices of public health education; strategic and operational planning, budgeting, and operations; information research, quantitative methods for assessing effectiveness; community assessment and resources; group dynamics and behavior; population based health strategies; health care practices and principles; human behavior and techniques for effecting behavior change; and grants management.
  • Skill in health program planning, implementation and evaluation; development of goals and objectives; community assessment; technical and grant writing; public speaking and making presentations to staff and public; meeting management, time management and organizing; and in the use of standard MS products, presentation and desktop publishing software.
  • Ability to prepare news releases, brochures, teaching materials and attractive, informative displays for health fairs and other public presentations; create successful grant applications and manage grants; work independently and as a team member; and create and manage effective interpersonal and interorganizational communication and coordination networks.

Preferred Qualifications:

  • Master's level education in Public Health or Health Education/Promotion or related field, and/or equivalent training/experience to fulfill requirements of position.
  • Previous experience in public health and the assessment/improvement process preferred.
  • Certified Health Education Specialist (CHES) for Health Educators preferred.
  • Experience working in community engagement.
  • Strong understanding of and experience working to address the social determinants of health.
