PACE Physician Assistant - Primary Care/Geriatrics
***$6,000 Sign-on Bonus***
We are looking for a bright, motivated and skilled PA to join our provider team at TRU PACE.
Schedule: Full-time, Monday-Friday 8am-5pm. We anticipate 3-days per week in office at our PACE facility in Lafayette, CO. 1 day per week visiting patients in facilities and their homes. 1 day per week working from home.
On-call responsibilities: Our PAs are "on-call" one day per week during their regular work week to assist our RNs with patient facing needs as they arise. Our PAs also rotate the weekend on-call schedule every 6 weeks. It's our goal to provide our team with a work/life balance.

JOB SUMMARY:
The Physician Assistant works in collaboration with physicians and interdisciplinary team to deliver primary care and other health services to participants enrolled in PACE.
The PACE Physician Assistant will work in the community making visits at patient homes, nursing homes, and assisted living facilities, our Inpatient Care Center (IPU), as needed to meet the needs of PACE participants, and Hospice & Palliative Medicine patients.
EDUCATION & REQUIREMENTS:
- Master’s degree in Physician Assistant Studies
- Physician Assistant license in the state of Colorado.
- Controlled substance license with current DEA license number.
- Eligible or currently possess a Medicare/Medicaid UPIN number
- Eligible or currently credentialed at local hospitals.
- Eligible to bill insurance for palliative care and symptom management visits.
- CPR Certification.
- One or more years of working experiences as a PA
- Experience working with frail/elderly preferred
- Experience working with individuals with mental health needs desirable.
- Knowledge of Medicare, Medicaid and hospice standards of practice.
- Excellent interpersonal communication skills.
- Must successfully pass agency required background checks and drug test.
- Must have $1M/$3M professional liability insurance and TRU will reimburse the premium costs
- Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage), and have reliable transportation.
- Must comply with agency health screening requirements, including medical clearance for all communicable diseases prior to evidence of direct participant contact, and proof that all immunizations are up to date prior to direct participant contact.
- Must be able to demonstrate computer competency.
ESSENTIAL DUTIES:
- Provides diagnosis, treatment and management services for PACE participants and hospice patients as directed.
- Participates actively in interdisciplinary team conferences, helping to develop and revise participant/patient care plans and acting as a medical resource in determining approaches to individual participant/patient care.
- Responsive to provide urgent visits in the clinic or in the participant home when required during work day and when on call during the day or on the weekend.
- Works in a safety conscious manner which insures that safe work practices are used in order not to pose a risk to self or others.
- Utilizes standard precautions in clinical practice.
- Understands the PACE model of care and actively works to implement participant driven care.
- Attends medical services staff meetings
- Meets with the Medical Director and/ or their physician supervisor for periodic formal review, at a frequency determined by the conditions of their physician assistant license in the state of Colorado.
