Role: The post is a fixed term 1 year contract, full-time working 35 hours per week.

I am looking to speak to someone who has experience in working as a Social Worker in a Primary Care role.

About you:

• You will evidence that you are a problem solver, confident about making important decisions, underpinned by a genuine passion to work collaboratively with people of all ages to meet their goals and aspirations.
• You will have strong person-centered assessment skills with a clear focus on relationship based and solution focused practice
• You will have excellent communication and listening skills.
• Be passionate at making a positive difference
• Key knowledge and application of the relevant legislation spanning the age groups of children and adults
• Be able to use a creative and bespoke approach to enable the client to access correct benefits, resources along with provision of clear and accurate advice and guidance

Essential Requirements:

• A recognised qualification as a Social Worker
• Registration with CORU
• Previous experience working in a Primary Care setting
• Knowledge of the relevant legislation spanning the age groups of children and adults
• A full driver's licence with access to a car
• Available to start work as soon as possible
