Job Summary:

Assessment and Referral Coordinators have the unique opportunity to help hundreds of people in need each month from the comfort of their own office, with a window! We are the point of contact for callers wanting to obtain services at Wesley Family Services. Daily duties include everything from determining if the service they are requesting is appropriate, to checking insurance/eligibility, to scheduling appointments, and entering their information into our system. If we are unable to bring them into our services, we provide them resources and guidance on how to obtain services elsewhere. Central Access is also the point of contact for internal and external staff who may have questions or want to make a referral. Monday – Friday Work Schedule: 4 days a week @ 8:30 a.m. – 4:30 p.m. / 1 day a week @ 9:00 a.m. – 5:00 pm. (Schedule can be flexed for doctors’ appointments, vacations, etc.)

Qualifications:

Education: Minimum of Associates degree, Bachelor’s degree strongly preferred, in a human services or related discipline from an accredited institution.

Experience:

  • Minimum one year experience in a human services field, preferably in a case management or admissions role. Two or more years strongly preferred.
  • Strong customer service and communication skills.
  • Competent with the Microsoft Office Suite, especially Outlook and Excel.
  • Experience networking and building relationships with referral sources, stakeholders, and families preferred.
  • Experience working with electronic medical record software preferred.
  • PA Child abuse, PA Criminal, and FBI clearances

Job Duties include:

-Answers incoming calls for individuals seeking services within Wesley Family Services

-Utilizes clinical judgement and screenings to determine if the program requested is appropriate or if a higher level of care is needed

-Assist callers in determining what programs are available within Wesley Family Services

-Transfers callers to the appropriate department when necessary

-Checks insurance and eligibility criteria

-Helps families understand their insurance and explain how to apply for Medical Assistance (MA)

-Schedules families for new patient appointments/intakes

-Inputs new clients in our Electronic Medical Record (EMR) to start a service

-Updates existing client records in our EMR when they are adding a new service

-Assists internal and external staff members when they have questions or want to make a referral to our programs

-Responsible for walk-in crisis and crisis calls (1 day a week from 9:00a.m. – 5:00 p.m.)

-Completes forms as per agency, state, county, and federal regulations governing situations, which indicate an at-risk situation.
