Basic Function: Under the supervision of the Director of Nursing, provides nursing services, staff training, consultation, health education, and health care coordination for clients living within a community residential setting.

Principal Duties/Responsibilities:

  • Provides health evaluations to persons supported in the residential programs.
  • Obtains and evaluates all medical records.
  • Monitors and updates physical examinations, vision and hearing examinations, immunization records, and medical treatment plans.
  • Monitors health records of clients to ensure that files are current and appropriate follow-up being implemented.
  • Records significant medical updates for all clients IHP’s.
  • Participates as a member of the IDT at annual staffing.
  • Provides medication monitoring to individuals receiving services. This includes completion of initial and annual updates of the Nursing Services Packet.
  • Provides training to designated staff in medication administration.
  • Provides consultation to staff on healthcare-related issues, medications, health screening, and any other health-related issues.
  • Must be knowledgeable of and comply with the confidentiality provisions of HIPAA.

*May be required to serve on health-related committees. *May provide individual, group or family counseling as part of an IHP where physical issues are presenting problems.

*Performs other duties and responsibilities consistent with community health nursing as assigned.

Mental/Physical Demands: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients or employees of the organization. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand, walk, and sit. The employee is occasionally required to use hands to finger, handle, or feel, and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

Environmental Conditions: The noise level in the work environment is usually moderate. Work is performed in a residential or office environment.

Specifications:

  • Position requires a current, valid license as a Registered Nurse in the State of Illinois; two years of successful direct nursing with Developmentally Disabled adults. It is desirable that the individual have the requisite training and experience to qualify as a certified developmental disabilities registered nurse (CDDN).
  • Demonstrate the experiences, beliefs, attitude and awareness that indicate cultural sensitivity to the client population served.
  • Ability to coordinate effectively with other team members, other Agency resources and outside services in addressing client needs.
  • This position requires the use of a personal auto, valid driver’s license and a motor vehicle report acceptable to the agency’s broker as well as having proof of current vehicle insurance. Must successfully complete criminal background check as determined by the State of Illinois Department of Human Services. Must clear a registry verification with the Illinois Department of Public Health.
