The Position

As a home care nurse, your responsibilities will include (but not be limited to):

Provide limited term skilled nursing services to individuals returning home from higher level care facilities such as acute and sub-acute rehab centers.

Assess, generate, and evaluate patient care plans and patient progress.

Liaise with other members of each patient's multi-disciplinary care team (e.g. Physical therapists, social workers, etc.)

Record patient progress in HCHB via tablet.

Attend regular staff meetings.

Job Types: Full time and Per Diem

Requirements:

Current NJ RN License

Reliable transportation, Driver’s License, and Insurance.

Strong interpersonal and customer service skills.

Ability to demonstrate knowledge of electronic documentation (HCHB or others)

Bilingual a plus

Additional training certificates a plus.

Preference for nurses with prior Home Health experience including: Psych, Vent, Peds, Admissions
