Hospice of the Piedmont is seeking a full-time RN Case Manager toprovide and coordinate compassionate and meaningful care to patients and their caregivers during the end stages of life in Charlottesville, VA, and counties to the west.
Duties include but are not limited to providing nursing evaluations to new admissions, regularly re-evaluates patient needs, consults with the patient’s physician, completes bedside documentation using the EMR, coordinates and manages services offered by the health team members, and helps ensure the final chapter of the patient’s life is one of quality, compassion, and dignity.
The ideal candidate will have Two years of relevant nursing experience; demonstrated ability to manage all aspects of terminal care and health team service coordination. Graduation from an accredited nursing school and a valid, unencumbered license as a Registered Nurse in the Commonwealth of Virginia.
