Job description

- Coordinates program planning, implementation, and evaluation in area of accountability in collaboration with members of the health care team. Identifies opportunities for philanthropic support to support program objectives. Acts as patient advocate, crossing multidisciplinary lines to meet patient needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
- Assesses the patient/significant other, health care team, and community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
- Develop and maintain network with internal and external partners to support programming. Develops support systems for patient population and coordinates program with other system-wide/community support programs. Participates in Hospital/Community committees as member and consultant on quality care issues.
- Initiates performance improvement studies in order to evaluate and measure the outcome of services. Make recommendations to improve the quality of services provided.
- Coordinates the delivery of patient care within Program services related to area of specialty.
- Performs and provides designated activities related to area of specialty.
- Maintains the documentation and record organizational and external agency requirements for services provided.
- Oversees selected staff as assigned by manager to coordinate program services.
- Bachelor's Degree or equivalent related field
- Preferred Master's Degree or equivalent related field
- 2 years of relevant experience related experience
- LIC-Registered Nurse (RN) - State of Michigan or
- LIC - Licensed Practice Nurse (LPN) State of Michigan
- CRT - Basic Life Support (BLS) - American Heart Association 90 days
