Job Description:
Responsible for delivery of social services to residents and their families. Responsible for consultation and direction of Social Service in the skilled nursing facility. Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling, recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates CQI activities
Responsibilities:
- Interpret social, psychological, and emmotional needs of the resident/family to the medical staff, attending physician, and other resident care team members
- Participate in discharge planning and make such plans available to the Administrator as required
- Provide written and or oral reports of the Social Work programs as required
- Attend and participate in the QA Committee functions as required
Requirements/Qualifications:
- Must posses a Master's Degree in Social Work or related field of study
- Must have, as a minimum, three years' experience ina Social Work capacity at a hospital, skilled nursing care facility, or other related medical facility
