Cedar Creek Hospital is seeking a talented and dynamic Social Worker/Therapist to join our team of dedicated healthcare professionals!
This is a full-time position working dayshift Monday-Friday.
Cedar Creek Hospital of Michigan opened in 2017, as a beautiful and state of the art facility. Cedar Creek provides evidence-based treatment to adolescents and adults struggling with mental health issues. Cedar Creek Hospital invests in our patients by offering an integrated medicine approach that provides the latest therapies including yoga, mindfulness, aromatherapy, music therapy and nutrition. As part of our success, growth and innovation in the field we are seeking qualified applicants for an Adolescent Virtual Partial Hospitalization Program PHP Clinical Social Worker position.
Position Summary:
The Social Worker completes psychosocial assessments, treatment plans, therapeutic group programming and discharge planning services for adolescent patient in virtual PHP programming. This role will also facilitate family sessions as needed.
Location: This position is an on-site role located at Cedar Creek Hospital. 101 West Townsend Rd, St. Johns, MI 48879

Clinical Social Worker (LMSW, LLMSW) Job Requirements:
- Current License in the State of Michigan as a LMSW or LLMSW.
- Adolescent Treatment Experience Required
- Master's level preparation from an accredited school.
