The Supportive Services Navigator provides a myriad of services to individuals and families experiencing housing instability or homelessness in Duval, Baker, Clay, and Nassau counties. The position links clients to effective, client centered services in their community. The overarching goal of the position is to provide a path for clients to attain self-sufficiency. The Supportive Services Navigator is responsible for educating, coordinating, and connecting clients with appropriate community services specific to their needs. Working closely with case managers to guide clients through their housing crises, the Supportive Services Navigator provides individualized support and advocates on behalf of the client. The Supportive Services Navigator creates and updates service plans including client dictated S.M.A.R.T. goals, monitors goal progression, and determines case exit.
Primary Responsibilities
- Educate the client on service options specific to their needs and makes the appropriate referral within Catholic Charities or to an external partner providing the required service.
- Analyze information obtained from client discussions to identify the needed wrap around services and create service plans to ensure services are accessed.
- Identify issues/barriers that hinder client service experiences; assists clients in resolving and mitigating barriers by identifying available alternatives.
- Maintain accurate and current detailed support services file on each partner for purposes of developing a robust catalog of providers for client referrals.
- Research new partnerships and solidify agreements with the stakeholders for the benefit of evolving service alternatives.
- Work closely with the Catholic Charities departments and teams to understand internal service options and goals and recommend potential new services that align with Catholic Charities mission and vision.
- Network with other non-profit partners and attends local partner meetings for referral services not provided by Catholic Charities
- Work as a liaison between educational institutions, healthcare agencies, elderly care facilities and populations, women’s centers, homeless shelters, and any other entities that assist underserved populations and those in desperate need of services.
- Provide individuals with travel training and/or assistance with public transportation.
- Submit timely, complete, and accurate documentation of each client contact and service provided.
- Maintain clients’ records by creating case notes, logging events and progress in a timely manner.
- Assist with solving immediate crises and facilitate client accessibility to basic needs such as food, shelter, health, and transportation.
- Provide informational training, financial literacy, budgeting and money management training, and assist clients with problem-solving.
- Objectively help clients articulate their thoughts using proven interviewing techniques. Such techniques may include asking open-ended questions, asking for clarification and helping a client make connections with information provided and documentation.
- Exhibit sensitivity to the service population’s cultural and socioeconomic characteristics.
- Utilize active listening and reflective listening techniques, so clients feel heard and understood.
- Maintain up to date and accurate cases files and databases, in accordance with HIPAA and confidentiality requirements.
- Other duties as assigned.
Other Tasks and Functions
- Attends Catholic Charities Jacksonville staff and departmental meetings
- Trains on a variety of workplace topics via the Relias, online class system
- Participates in and hosts in-service activities, continuing education opportunities, conferences, and workshops to supplement knowledge and enhance professional development and life-long learning knowledge
- Learn and understand grant and program deliverables, criteria and needs to better service our clients
- Become the subject matter expert on community wide supportive services and their accessibility.
Job Type: Full-time
Pay: $17.00 - $19.00 per hour
