Job Description:
SUMMARY OF POSITION
Centerstone Healthier Beginnings an accredited Healthy Families America program offers home-based services to expectant families and new parents, beginning prenatally or shortly after the birth of the child and continuing until the child's 5th birthday. The program is a comprehensive prevention program that focuses on the safety of the child while at the same time supports the family. Families who participate in the program are offered long-term in-home services until the child is in school. Family Support Worker (FSW) helps the family meet its immediate needs; offers emotional support to help the new parents cope with the stresses of parenting a new born, provides prenatal and other health education. Assist in establishing contact with community resources.
ESSENTIAL DUTIES & RESPONSIBILITIES
  • Determine eligibility in accordance with guidelines.
  • Provide continuing child/family assessment as required.
  • Make referrals to community based services for non-eligible families.
  • Maintain accurate, complete and confidential records that reflect screening and assessment outcomes.
  • Prepare and submit reports as required.
  • Attend trainings and regular meetings as needed.
  • Work with community services providers, the medical community and hospitals to assure that families are given the opportunity for screening and assessment.

Education Level
Bachelor's degree required.

Bilingual (English / Spanish) Preferred.
