Job Title: Family Health/Mental Health Advocate
Location: Lawrence, MA
Position Type: Full Time, Salary Non-Exempt
Salary: $48,000 - $52,000
Overview:
Casa Nueva Vida, a subsidiary of Heading Home, Inc., is in seek of a Family Health & Mental Health Advocate that will assist in linking families to health and mental health services. This person will work closely with the Emergency Assistance (EA) Family Shelter program to ensure families are receiving the appropriate health and mental health care needed to address barriers in these areas that may be contributing to homelessness. This includes assisting clients with access to insurance, medical facilities, and arranging for transportation to appointments when appropriate.
Responsibilities:
- Collaborate with referred clients and program staff to assess health and mental health needs.
- Facilitate connecting clients to needed services.
- Assist clients in applying and obtaining medical insurance as needed.
- Assist clients, as appropriate, in getting to scheduled appointments for medical, dental, substance use, and mental health services.
- Work closely with EA family shelter staff to collaborate and communicate regarding services being arranged for clients to ensure positive outcomes
- Follow up with clients and staff to ensure client is receiving health/mental health services.
- Record client data as directed by supervisor or Director of Data and Research.
- Complete case notes identifying interventions provided and client progress in obtaining and maintaining needed services, in a timely fashion
- Act as a liaison and advocate between health and mental health service providers and client.
- Attend trainings and certifications as required for program compliance.
- Take on additional tasks as assigned by Supervisor.
- Perform other duties as assigned.

- Minimum two years’ experience working with people experiencing homelessness or low- income populations
- Thorough knowledge of how to apply for and obtain insurance benefits
- Thorough knowledge of community resources in medical, dental, substance use and mental health services and how to connect clients to these resources
- Ability to advocate as appropriate with agencies and service providers to ensure that clients receive all necessary supports to encourage housing and sustained tenancy
- Sufficient knowledge of public transportation system to direct clients to appointments when applicable
- Able to work effectively independently and in a team setting
- Adequate computer skills to navigate online resources, enter data and prepare documents and/or reports
- Bachelor’s degree in Social Work or related field; Master’s degree preferred; any combination of education/experience that equips applicant to carry out the above listed responsibilities. Master’s degree holders who are seeking licensure may be able to receive supervision towards licensure hours

Cubicle space within large office building; prolonged sitting at a desk and working on a computer, ability to climb up to three floors of stairs, ability to lift and carry up to 20 pounds.
