ReMed, a Collage Rehabilitation Partners is looking for an Activities Specialist to join our outstanding residential teams in the West Chester and Paoli, PA locations. For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries. Our CARF accredited, post acute residential, vocational and outpatient programs are offered in the Philadelphia, Pittsburgh regions of Pennsylvania, New Jersey, Louisiana and Maryland, Georgia, North Carolina, Virginia, Texas, Utah, Colorado, and California
Responsibilities:
Job Summary
This position assists in the development and implementation of a comprehensive recreational program, which includes both group and individual activities in the residence and the community. This role oversees the individual recreational programs of assigned clients; completes initial assessments, monitors monthly progress and prepares all necessary discharge plans.
Essential Duties & Functions
- Demonstrates ability to support clients in difficult situations (e.g. refusal of activities, medications, and strategies).
- Demonstrates ability to identify changes in clients’ behavioral, medical, emotional, cognitive, and physical condition and alert applicable team members for appropriate care as needed.
- Works directly with the client and provides minimum to maximum assistance and/or supervision to promote the client’s success in daily living tasks, recreational activities and vocational/educational pursuits as identified by clients’ treatment plan (e.g. residential/hygiene routines, meal prep and planning, apartment/room upkeep, transportation training, pedestrian safety, money management/budgeting systems, walking, PT exercises, visual exercises).
- Demonstrates proficiency in completing all tasks necessary to support the client’s treatment plan. (e.g., direct care, med admin, transportation, job coaching, transfers, and med procedures).
- Demonstrates proficiency in leisure/recreational principles (e.g. individual and group leisure activities, special events, overnight trips, sober leisure lifestyle, and social skill development).
- Works in collaboration with the Recreation Therapist to understand, monitor and provide feedback on strategies and leisure activities related to a client’s individual recreation plan in the residence/apartment and in the community.
- Works with the Brain Injury Specialist to ensure clients’ individual recreation plans are scheduled and supported by appropriate staff.
- Takes responsibility for communicating all necessary information to appropriate team members, including being prepared for discussions at rounds.
- Takes responsibility for seeking out current information regarding client’s program and ReMed information before start of shift (e.g. staff comm., rounds summaries, meeting minutes, client strategies, postings, memos)

Requirements for the position include:
- A high school diploma is required; an associates or bachelors degree is preferred
- Experience/willingness to facilitate recreational activities in the residence and community settings
- Exellent planning, documentation, organizational and time management skills.
- The ability to communicate with families, professionals and coworkers.
- The ability to demonstrate positive client interaction skills is essential.
- A valid driver's license, insurance and responsible driving behaviors must be maintained. Ability to drive to homes or work sites of some referred outpatients as well as to support medical appointments in the community.
