The Gándara Center provides residential, mental health, substance abuse, and preventive services for children, adults, and families across the Pioneer Valley and eastern parts of Massachusetts. Duties and Responsibilities:
- Provides services in a manner that is strengths-based, family-driven, youth guided, and culturally.
- Works with the Program Director, Clinician, and other members of the treatment team to ensure overall professional services are delivered.
- Solicit/receive referrals, engage and place YAs in a timely.
- Work with the program clinician to complete substance abuse, educational, job readiness assessments with participants and develop treatment plans with Young Adults to achieve
- Provide in-home individualized case management, advocacy, safety planning, and planning for housing, employment, and/or education to Young Adults, including, but not limited to, the following activities: link Young Adults to community resources to ensure mental and behavioral health needs are being managed, assist Young Adults in process of obtaining housing, working with landlords, and monitor progress with tools provided and adjust as needed.
- Completes all required reports and tracking tools for Young Adults, funding sources and the.
- Perform other tasks as determined by the YATC Program Director, Clinician, or other Department/Agency/DMH.
- Informs the Agency of all emergencies or critical incidents.
Minimum Requirements:
- Bachelor's degree in a human service
- Must have vehicle transportation, proof of valid driver's license, a good driving record, and vehicle
- Must have an acceptable CORI background
- Ability to work effectively with diverse populations and community agencies: School Dept., DYS, Juvenile Court,
