What You'll Be Doing:
The Core Measures Manager will be responsible for performing detailed medical record review and abstraction of clinical data required for quality measures reporting and/or departmental registries. Participates in hospital wide activities or projects. Supports patient safety and quality improvement efforts through accurate reporting of compliance to defined metrics and collaborates with appropriate committees, and department team members to facilitate improved patient outcomes. Serves clinically and administratively as a resource in applying management technique, systems, functions, and regulation interpretation.

The successful candidate will be able to:

Manages data and information in a manner that ensures integrity, accuracy, and confidentiality.
  • Performs medical record review and abstracts large volume of clinical case records to support the quality improvement and regulatory reporting requirements.
  • Analyzes data routinely assessing the integrity of data and information across the organization and initiates process to resolve data issues.
    • Makes recommendations based on recognition and analysis of gaps in documentation.
    • Identifies opportunities for performance improvement based on reliable data trending.
  • Maintains confidentiality of Performance Improvement data and information and manages data and information consistent with the organization’s confidentiality policy.
  • Assesses potential problems affecting the data collection process and coordinates follow-up of problem resolution with the team and/or director.
  • Provides timely data reports and variance information to departments and/or committees, meeting all deadlines.
Concurrently
  • Tracks Core Indicator Measure patient charts to ascertain compliance with Core Indicator Measures and documentation of same.
  • Provides concurrent feedback on identified areas of non-compliance.
  • Content expert on all Core Indicator Measures and serves as a resource to hospital staff / physicians.
Retrospectively
  • Abstracts Core Indicator Measures, inputs data into the designated software prior to the due date.
  • Maintains at least a 75% accuracy rate on CMS validation audits.
  • Identifies trends and brings to the attention of the Assistant Director / Director of Performance Improvement.
  • Provides information to hospital and medical staff regarding Core Indicator Measure outcomes and improvement activities.
What We're Looking For:
Full-time, Day shift, 8 hours
  • Bachelor’s degree required; Master’s Degree preferred.
  • Current New Jersey/Multi-State Professional Registered Nurse Licensure.
  • 2 - 4 Med/Surg nursing with expertise in Cardiology, ICU, Neuro setting required.
  • 5 - 8 years prior experience with Core Measurement Management preferred.
  • Knowledge of Federal, State and Joint Commission regulations required.
  • Detailed-oriented, committed to high quality data abstraction, meets quality and productivity standard. Ability to maintain an accuracy score which meets department requirements.
  • Ability and willingness to be cross-trained and become proficient in multiple clinical domains requiring clinical abstraction services.
  • Strong verbal and written communication skills.
  • Proficiency in Microsoft Office applications.
  • Experience working with Meditech, highly preferred.
  • Ability to figure things out on your own, confidence, and competence to communicate with all levels of management
