ABOUT US

UP Behavioral Health deploys medical teams that specialize in addiction medicine, addiction recovery and pain medicine. We provide service to detox residential facilities, intensive outpatient care programs, partial hospitalization, sober living, and beyond. Our team provides top level addiction expertise with a holistic approach and individualized treatment plan that treats the whole patient which includes internal, addiction, psychiatric and pain medicine.

JOB QUALIFICATIONS

• A Master's degree from an accredited school of social work (Required)
• Possession of a current clinical social worker license issued by the California Board of Behavioral Science Examiners and three years of post-master's experience - three of which should include administration, supervision and/or consultation in a health care setting; or an equivalent combination of education and experience; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
• Excellent communication and organizational skills
• Computer Skills

RESPONSIBILITIES AND DUTIES

As a Licensed Clinical Social Worker, you will be formulating and implementing psychosocial treatment; engage in special projects through all of UP Medical's business lines such as:

• Internal/external referrals.
• Conduct pre-assessments (biopsychosocial, PHQ 9, GAD 7, audit C).
• Manage with considerable independence between UP Medical business lines as a patient advocate.
• Working with clinical teams to help with patient compliance to treatment plans and ensure as few interruptions in their patient journey as possible.
• Utilization of Management/Care Management.
• Communicate with physicians to report patient progress, needs, concerns, and potential and actual problems.
• Maintains accurate and timely records including clinical and progress notes according to department policy.
• Work closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
• Develop working relationships with leaders in community resource groups, representing UP Medical at meetings and on various committees that address the social needs of patients.

Job Type: Full-time

Pay: $75,000.00 - $90,000.00 per year
