DESCRIPTION

This position will work in UCSD Health's Population Health Services Organization (PHSO). PHSO supports and work in in collaboration with ambulatory care providers to help with our shared mission of population health for the patients we serve. This NP will perform home visits for UCSD at Home visiting senior home confined patients to help them with medical management so they do not have an unnecessary admissions. The NP works with the PCP to ensure updates, communication and as a resource as needed. Ideal candidate would have experience working in patients homes and working with seniors. Geographic area may vary, a UCSD leased car is provided along with an NP bag with supplies as needed to support in home patient assessments. Ideal candidate will be a seasoned NP with strong technical, clinical and time management skills and a passion for senior patients and helping them set goals and a care plan to ensure they are safe at home.

MINIMUM QUALIFICATIONS

  • Must be a graduate from an accredited nurse practitioner program.
  • California Registered Nurse (RN) license, California Nurse Practitioner (NP) license, and Furnishing License Certification.
  • Current NP Specialty Certification from one of the following Nursing Boards: AANP, ANCC, NCC, PNCB, AACN.
  • BLS/CPR certification at time of hire with commitment to get BART within six (6) months of hire date.
  • Minimum two (2) years of recent working NP experience in primary care or home care. Due to advanced practice credentialing eligibility, clinical advanced practice experience must be within the last two years gained through school or work experience.
  • Must possess excellent interpersonal and communication skills.
  • Ability to work autonomously.

PREFERRED QUALIFICATIONS

  • ACLS and/or ART.
  • Previous experience doing home visits.
  • Experience working with senior patient population.
  • Strong knowledge and comfort with technology.
  • Understanding of Medicare and associated guidelines.
  • Well-developed clinical competency in general practice setting.
  • Bilingual English/Spanish.

SPECIAL CONDITIONS

  • Conducts face-to-face medical visits at the home of the patients for routine visits, acute illness, and post-acute discharge for patients with complex medical conditions.
  • This role will provide medical care to both post-acute and long-term patients at a high-quality skilled nursing facility.
  • Must be able to obtain UCSD Medical Staff privileges and maintain during employment.
  • Must be able to work various hours and locations based on business needs.
  • Must have access to reliable transportation.
  • Willing to travel and work in a fast-paced environment independently.
  • Employment is subject to a criminal background check and pre-employment physical.
  • Selected candidate may be hired at the NPIII level pending review and approval by the Advanced Practice Council.

Pay Transparency Act

Annual Full Pay Range: $149,229 - $208,842 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $71.47 - $100.02

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
