Job Title: Patient Services Coordinator (Hospital Social Work)


Roles & Responsibilities:

The Patient Services Coordinator (Social Work) plays a crucial role to assist the Social Worker by performing specific tasks in order to help facilitate throughput and discharge. The incumbent serves as liaison for patient care activities for the Social Work department and works to provide and improve continuity to the patient’s experience.

Duties and Responsibilities:
1. Assists patients/care partners with concrete needs as follows:
o Transportation
o Uncomplicated DME
o Medication pickup and delivery from internal/external pharmacies
o Parking passes from Security
2. Facilitates tasks related to discharge including, but not limited to, distribution of MetroCard, petty cash and patient clothing from the Social Work central office.
3. Works cohesively with Social Worker in obtaining community resources and support services (e.g. Meals on Wheels, food pantries, etc.).
4. Collaborates with Social Worker, to resolve issues impacting patients’ care progression and will assist with resumption of home care services for MLTC patients.
5. Monitors and updates the medical record in accordance with standards set for the Department of Social Work and/or regulatory agencies (when applicable).
6. Under guidance of Social Worker, assists with referrals to REAP through Epic.
7. Under guidance of social worker, assists with referrals to NYLAG, following the established protocols.
8. Provides front-end coverage as needed; performs specific assignments relevant to particular programs.
9. Provides recommendation for process/service improvement to promote patients satisfaction and works with Social Worker to address issues.


Requirements:

Education:
Bachelor’s degree from an accredited college or university.

Experience:
Related experience in the social service and/or health care field preferred.

Compensation

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $40.00 - $60.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
