Purpose:
The Registered Nurse provides oversight and direct health care service to women and children residing in Residential Treatment Program for Women and Children in South Los Angeles and Outpatient Program. The Nurse upholds the JWCH mission to improve the health status and wellbeing of under-served segments of the population of Los Angeles County. This position is funded through three contracts including Los Angeles County Department of Public Health Substance Abuse Prevention and Control (SAPC) Sexual and Reproductive Health (SRH) and SAMHSA Perinatal and Parenting (PPW) grant.
Duties and Responsibilities:
- Performs routine care functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities
- Responsible for adhering to County and Federal contract conditions
- Responsible for overseeing coordination of Physical Examination and TB compliance for each patient
- Logs medication, maintains Medical Administration Record, and audits MAR at prescribed intervals
- Organizes and maintains medication cabinet and patient medication boxes; to include removing medications left by patients and packaging them for proper disposal.
- Meets regularly with patients to review medications, side effects, appointments and refills needed.
- Establish working relationships with Wesley Clinic Administrators
- Oversees and determines Medi-Cal eligibility and coordinates with Enrollment Specialist for out of county transfers
- Assesses if patient has a medical provider and make change to Wesley Health Centers as appropriate. Assists in completing enrollment paperwork
- Schedules appointments in Next-gen for psychiatric, family, prenatal and postpartum health care and other medical services
- Facilitates the coordination of Smoking Cessation Services
- Coordinates and delivers Sexual and Reproductive Health services per County SRH contract
- Responsible for tracking Sexual & Reproductive Health (SRH) services and submitting monthly report
- Coordinates with internal and outside agencies for HIV Education, HIV and STD testing and SRH services
- Counseling on risk of HIV, Hepatitis C and other communicable diseases.
- Oversees urinalysis testing, submission and results
- Conducts intake assessment, including Health Questionnaire, Women’s Health History and Reproductive Health.
- Enters assessments and notes into Electronic Health System (EHR), SAGE in compliance with contract standards
- Oversees and conducts COVID-19 test with new patient admissions and ongoing testing as needed. Keeps an inventory of rapid test
- Prepares for and participates in weekly Case Review
- Communicates regularly with Medical Director
- Attends SAPC Providers Meeting for Perinatal and Parenting Women (PPW) and Sexual & Reproductive Health (SRH)
- Coordinates with Chef on dietary restrictions with any patients and food needs of children
- Coordinates with Program Administrative for appointment transportation needs
- Coordinates care with Medication Assisted Program, as well as other agency departments (HIV testing, Family Planning, Dental and Pediatric)
- Maintains Incident Log when Paramedics are called to the facility, and follows through when patient is admitted to the Hospital and reviews discharge paperwork upon return
- Conducts Health Education Seminars on topics such as Harm Reduction, Nutrition & Addiction, Family Planning
- Conducts staff seminars on health and wellness related topics.
- Enforce all COVID-19 safety measures
- Supervises a Medical Assistant or LVN
- Performs other duties as assigned.
Position Qualifications:
- Current and valid California Registered Nurse License.
- Current Basic Life Support and First Aid card from the American Heart Association.
- A valid California Drivers License.
- Able to work with challenging and resistant populations.
- Ability to communicate effectively orally and in writing.
- Experience in working with patients and staff from diverse socio-economic, ethnic and cultural backgrounds.
- Cultural competent working with minority, low income and vulnerable women.
- Able to work Holidays and other nontraditional work days and hours.
- Bilingual capabilities in English and Spanish preferred.
Skills and Knowledge:
- Knowledgeable of alcohol and substance addiction, systemic trauma, sexual and reproductive health, smoking cessation, medication assisted treatment and child development
- Knowledgeable of mental illness diagnosis and psychotropic medications treating mood disorders, schizophrenia and anxiety disorders
- Proficient in Microsoft Programs
- Proficient in managing Medical Administration Record
- Excellent documentation, oral and written communication skills
All JWCH, Wesley Health Centers workforce members must be fully vaccinated against COVID-19 as a condition of employment.
