Job description
Provides services to injured workers by coordinating return-to-work activities with all interested parties and assisting medical providers, injured workers and employers toward a safe and timely return to work plan, ultimately reducing unnecessary work disability time. Provides services to injured workers by providing resources, coordinating efforts, and setting recovery-time goals. This is a grant funded initiative with a plan to extend beyond grant duration based on program growth and success.
Essential Job Functions
Coordinates the full cycle of the injured patients’ case from onset to completion.
Advocates for all parties and serves as a liaison on behalf of the provider to ensure the injured patients’ get back to work in some capacity as soon as safely possible.
Contacts doctor/provider to determine patients’ needs; communicates with employers to inform them of what is acceptable light duty, so the patient is back to work and performing tasks that won’t exacerbate the injury.
Tracks, addresses, and communicates barriers that prevent a patient from returning to work and develops strategies to address the issues.
Assists providers in completion of return to work documentation.
Accesses and reviews provider documentation to establish patients’ severity level, review providers’ plan of care, and address any barriers to plan of care.
Ensures care coordination among providers, staff, and employers to improve clinical outcomes.
Works with the employer to identify ways to modify the job, the equipment, or the location, when necessary.
Collects and organizes data that monitors the effectiveness of the RETAIN health care delivery system.
Receives and reviews standard reports with other RETAIN team members to continue to improve the program’s efficiencies and promote success.
Participates in performance improvement activities and contributes to program development to facilitate accomplishment of departmental goals

Bachelors’ degree

Current RN licensure required.
Minimum Qualifications
Minimum Years and Type of Experience
Three (3) years of experience in either direct patient care, case management, return to work coordination, or occupational health care required.
